Anyone who lives within our practice boundary can register as one of our patients. If you are unsure if you live within our boundary, there is a map of the practice area in the waiting room that you can view, or please ask for advice from our receptionists.
If you wish to register, please see one of our receptionists who will give you a simple form to complete, We will need to have details of your previous or current GP, and your NHS number, as well as the documentation described below.

Each individual over the age of 18 will need to provide supporting evidence of identity which can include:
- National Passport
- Photo Driving Licence
- National Identity Card
- NHS Medical Card
- For children aged under 16 we require a copy of their birth certificate.
Only one adult family member need attend the surgery to register a family provided that evidence of identity is provided for each person, each adult individual has signed a registration form and the address details are the same for them all.
A photocopy will be taken of the identity evidence for each patient and attached to the registration form.
2. PROOF OF ADDRESS
This will verify that patients are not only living within our practice area, but that they are ordinarily resident in the UK and therefore entitled to receive free NHS treatment. This evidence should be:
- A utility bill
- Tenancy agreement
- Bank statement
- Mortgage statement
- Council tax bill
- Other official correspondence verifying address
